Fresh cucumber or a sprinkle of toasted sesame can add interest. Steer clear of the classic tuna-mayo, salmon and umeboshi centers, since those are the exact things pushing onigiri into high territory.
How to lower the risk
Buying onigiri pre-made from a chiller counter is the higher-risk path because you cannot see how fresh the fish was or how long it has been sitting. Making it yourself is the real fix: cook rice, let it cool just enough to handle, shape it, add a trusted filling like just-cooked chicken or nothing at all, and eat it soon rather than packing it for hours later. If you do carry it, keep it cold and eat it the same day.
